Change Control

How changes to a running system are logged and approved: rules, connectors and models, each signed by the role that owns it, in every Lyrion system.

Rule changes

  • Referral limit for commercial property raised to the new rate card, approved by the Head of Underwriting and recorded in the audit trail
  • Renewal letters on a renewals system now quote the reason for every rate change, approved by the Head of Customer Operations

Connector updates

Access and approvals

  • Approval routing now sends payments outside the matching rules to the Finance Controller instead of the team queue

Model changes

  • A newer model version is approved for document review and listed in the Model register
  • Outputs compared side by side on the same records before the switch, signed off by the system owner
  • Model register now records which processes each model may serve, and the region each one runs in
  • Previous model version kept available for rollback until the Head of Claims confirms the change
  • Prompt for the intake summary revised, reviewed by the claims team lead
  • Confidence threshold for automatic filing raised, so more cases are handed to a person
  • Model provider terms reviewed by legal and attached to the change

Approvals, access, and wording

  • Signing limits for the claims desk updated in approval routing

  • Access for two departed users revoked across every connector on the same day, confirmed by the security team

Reconciliation, intake, and sign-in

  • Removed a retired ledger account from the reconciliation rules, approved by the Finance Controller
  • Call transcription moved to a newer model on the model register and tested in the review environment
  • Intake no longer calls summary-model-v3, and claim notes and call summaries now use summary-model-v4
  • Multi-factor authentication enforced for every approver on the production sign-in page

Intake and review

  • Search intake records by policy number, approved for the service team by the system owner
  • The document review queue now shows the clause a decision was based on, beside the decision itself
  • Records created by the system carry the time zone of the office that owns them

Model governance

  • Document and image model entries now show who approved each model, and for which process
  • Each model entry shows the region it runs in and whether it may see personal data

Monitoring and alerts

  • Exceptions waiting longer than their agreed time are flagged to the team lead
  • Alerts for failed connector calls now reach your on-call engineer and ours at the same time
  • Daily checks compare system totals with the system of record and log any difference
  • Weekly summary of exceptions handed to a person, sent to the system owner
  • Alert thresholds reviewed with the operations lead and recorded as a change

Exception handling

  • Exception review on a renewals system now lists each case a person resolved, with the reason they gave and the rule it touched
  • A resolved exception can become a rule the system follows next time, once the system owner approves it and the change is recorded

Audit export

  • Export the audit trail to your own tools
  • Choose any date range, from a single day to a full year, so the trail matches your audit period

Change log

  • Every system now keeps a change log of rule changes, connector updates and model changes, each with its approver

Rating rules in review

  • New rating rules for small commercial risks tested in the review environment
  • Rules go live only after the Head of Underwriting approves the test results

Audit insights

  • Group the audit trail by approver to see who signed for what, across every connected system
  • Audit filters are saved with the review, so the same view can be shared with your auditors
  • Clearer labels: actions taken by the system and actions taken by a person are now shown apart

Connectors: telephony and email

  • Calls and emails from policyholders now logged against the right record, through a reviewed connector to the phone system
  • Retention for call notes set by legal

Review environments

  • Updated letter review queue with tracked changes, side-by-side drafts, and approval history
  • New document review queue for the claims team, approved by the Head of Claims
  • Test results from the review environment are attached to the change before it goes live

Residency: logs and backups

  • Logs and backups now stay in the same region as the data, set on the residency page and approved by the security team
  • Backups older than the retention period are deleted on schedule, and each deletion is logged
  • Region for each connector shown beside the connection, so reviewers can check it at a glance

Renewals model in review

  • Renewal summaries drafted with a newer model, reviewed before release
  • Clearer rejection notes: every draft a person rejects is logged with the reason

Draft and publish

  • Draft and publish: rule edits are saved as a draft that the system owner approves or discards, with draft and live versions clearly labeled
  • Suggested rules from resolved exceptions refine your system only after approval, and a new wording tab keeps standard clauses in one place
  • A new statistics tab charts exceptions handed to a person and approvals by role

Review environment v2

  • All-new review environment at /review: every change tested on real records, read-only, before it can reach production
  • Attach test cases by drag-and-drop, compare outputs against the live version, and record results with the approval_record field
  • Clearer results: every test shows the rule, the record and the expected outcome

Rule library and wording

  • 21 new standard clauses
  • Updated rule library with search, filters, and the approver for each rule
  • New multi-step approval for wording changes, available to counsel. You can also propose a change right from a flagged clause

Access review

  • Access rights reviewed across security, connectors, team, approvals, and audit trail, with service accounts listed on one page
  • Restrict a service account to specific systems of record when creating or editing it

Connectors: systems of record

  • Bring your own connectors, with your own authentication and your own network rules
  • Per-connector read and write controls, with read-only defaults and explicit write approval
  • New policy_lookup and ledger_lookup connectors, and handoffs to a named person when a record cannot be matched

Reconciliation and close

  • Reconciliation rebuilt as a single view that adapts to each reviewer's role
  • Month-end close shows unmatched entries instead of a single total, each routed to a person
  • Close checklist shows live status for each entity, and sign-off is recorded by the Finance Controller

Model register

  • Reviewed model register and new per-model approval records
  • Exact model versions, the region each one runs in, and the process each one is approved for, on each entry

Go-live and sign-in review

  • System moved to production after parallel running, signed off by the system owner and the security team
  • Reviewed sign-in page with your identity provider as the only source of access
